Personal Health Manager
It can be hard to keep track of all the information you receive about your diagnosis and treatment, but keeping this information organized can help you feel less stressed and more prepared to talk with your doctor. The ACS Personal Health Manager can help. Your Personal Health Manager content can be printed and kept in a 3-ring binder to take along to your appointments. Using tabbed dividers to separate each section will make it easier for you to find what you need.

Now you can get one-on-one support and the quality curated information you need in a new digital app for easy access, available anytime and anywhere. Speak with a trained ACS team member who can connect you to resources over the phone. For non-clinical support, you can also be matched with a trained volunteer who shares a similar cancer experience, including diagnosis, location, military status, race, and ethnicity. Cancer Survivors Network
Our Cancer Survivors Network (CSN) is a free online community where survivors and caregivers share their stories, ask questions, and get support from each other. With a chat room and more than 40 discussion boards, CSN allows you to connect with others who have a similar cancer experience. 24/7 Cancer Helpline
The American Cancer Society’s (ACS) cancer helpline provides 24/7 support when you’re dealing with cancer by connecting you with trained cancer information specialists who can answer questions about your diagnosis and provide guidance. You will be connected to American Cancer Society programs and services and provided with referrals to other national resources. Please call 1-800-227-2345 or visit www.cancer.org for a live chat. The ACS can assist in English, Spanish, and more than 200 other languages via a translation service.

Rides to Wellness
Rides to Wellness is a comprehensive non-emergency medical transportation program that provides mobility management, door-to-door service, and same day service to riders going to medical or other health and wellness-related appointments.

Patient Advocate Foundation
This independent division of Patient Advocate Foundation provides small grants to patients who meet financial and medical criteria. Grants are provided on first-come first served basis and are distributed until funds are depleted. Qualifications and processes for each fund may differ based on fund requirements.

Financial assistance for cancer-related costs such as transportation and child care. Our professional oncology social workers can find additional resources.
